Meropenem, an antibiotic is prescribed for the treatment of intra-abdominal infections including appendicitis, peritonitis, bacterial meningitis, nosocomial pneumonia (hospital-acquired), skin and soft tissue infections in adults and children.
Meropenem exerts bactericidal activity by penetrating into the microbial cells and by interfering with the biosynthesis of vital cell wall biochemicals that lead to cell death.
Plasma clearance of Meropenem is directly correlated with renal creatinine clearance rate. Impaired renal clearance of Meropenem is reported in kidney disorder patients and elderly patients. Intravenously administered Meropenem can be cleared by hemodialysis. No need for an adjusted dose of Meropenem in hepatic disorder patients.
The bactericidal activity of meropenem results from the inhibition of cell wall synthesis. Meropenem readily penetrates the cell wall of most Gram-positive and Gram-negative bacteria to reach penicillin-binding-protein (PBP) targets.
Meropenem should be administered to patients with allergy to Penicillin antibiotics, Cephalosporin, Imipenem and Meropenem. Meropenem is contraindicated in patients with renal diseases, brain tumor, head injury and seizures. An adjusted dosage may be needed in such patients.
History of hypersensitivity to carbapenem, pencillins or other β-lactam antibiotics; infants <3 months; renal insufficiency; neurological disorders; pregnancy, lactation.
Serum levels may be increased by probenecid. May reduce serum valproic acid levels; sub-therapeutic levels may be reached in some patients.
